Resumo:
Aim:
The present study aimed to characterize the morphological features of the mesial root of mandibular molars with a middle mesial canal (MMC) using micro-CT imaging.
Methodology:
A total of 469 mandibular molars with two roots and fully developed apices were randomly selected from a local tooth bank. The mesial roots with MMC (n=25) were evaluated for several morphological aspects, including the MMC configuration and the minimal periradicular dentin thickness measured 2 mm below the root furcation area.
Results:
The prevalence of MMC was 5.33%, with the confluent configuration being the most common anatomical variation. Most specimens with MMC had three independent orifices at the pulp chamber floor (n=16, 64.00%), and this canal was most frequently located in the coronal third of the root.
At the 2-mm axial level below the furcation, the average periradicular dentin thickness was 1.25 mm for the mesio-buccal canal (range: 0.71–1.61 mm), 0.99 mm for the MMC (range: 0.78–1.29 mm), and 1.22 mm for the mesio-lingual canal (range: 0.78–1.53 mm).
Conclusions:
The micro-CT analysis of the mesial roots of mandibular molars with an MMC revealed considerable variations in internal root canal anatomy. The confluent configuration was the most prevalent anatomic variation, while an independent MMC was observed in only one specimen. The lowest average periradicular dentin thickness at the 2-mm level below the furcation was observed in the MMC, measuring less than 1 mm. Clinicians should be aware of these anatomical variations before initiating root canal treatment to minimize the risk of iatrogenic complications.

Objetivo:
O presente estudo teve como objetivo caracterizar as características morfológicas da raiz mesial de molares mandibulares com um canal médio-mesial (MMC), utilizando micro-CT.
Metodologia:
Foram selecionados aleatoriamente 469 molares mandibulares com duas raízes e ápices totalmente desenvolvidos, provenientes de um banco local de dentes. As raízes mesiais com MMC (n=25) foram avaliadas em vários aspetos morfológicos, incluindo a configuração do MMC e a espessura mínima da dentina perirradicular, medida 2 mm abaixo da zona da furca radicular.
Resultados:
A prevalência de MMC foi de 5,33%, sendo a configuração confluente a variação anatómica mais comum. A maioria dos espécimes com MMC apresentava três orifícios independentes no pavimento da câmara pulpar (n=16, 64,00%), e este canal era mais frequentemente localizado no terço coronário da raiz.
Ao nível axial 2 mm abaixo da furca, a espessura média da dentina perirradicular foi de 1,25 mm para o canal mésio-vestibular (intervalo: 0,71–1,61 mm), 0,99 mm para o MMC (intervalo: 0,78–1,29 mm) e 1,22 mm para o canal mesio-lingual (intervalo: 0,78–1,53 mm).
Conclusões:
A análise por micro-CT das raízes mesiais de molares mandibulares com MMC revelou variações consideráveis na anatomia interna do sistema de canais radiculares. A configuração confluente foi a variação anatómica mais prevalente, enquanto um MMC independente foi observado em apenas um espécime. A menor espessura média da dentina perirradicular ao nível de 2 mm abaixo da furca foi registada no MMC, medindo menos de 1 mm. Os clínicos devem estar cientes destas variações anatómicas antes de iniciar o tratamento endodôntico, para minimizar o risco de complicações iatrogénicas.
